Opt-In Details for Health/Dental Insurance

You may opt-in to the plan during our regular opt-in periods (September and January). If you need to opt-in at other times during the year, please contact our office.

How to Opt-In

In Person: Come by the SGPS Office during office hours (see the footer on this page) and fill out an application form. We can only accept cash or cheque as forms of payment.

Out of town? For January 2012 - August 2012: download the opt-in form here. Complete the details and mail it along with your cheque payable to "The Society of Graduate and Professional Students".

Articling Students, Postdocs and RMC Students

The SGPS is pleased to be able to extend our Health and Dental Plan to you. To opt-in to our plan, please follow the details above.

Students That Have Graduated

If you were a member of the SGPS, you are eligible to opt-in to the SGPS Health and Dental plan for an additional year after graduation. To opt-in to our plan, please follow the details above.

Health and Dental Blackout Period

If you have elected to use the SGPS Health/Dental plan you are covered as of September 1. There is a blackout period. During this period, Green Shield Canada will not think you are a valid insurance holder. We have to work with Green Shield and Morneau Sheppell to add all the new students. This process, as you can imagine, takes a great deal of work and time. Please be patient. The blackout period should end by mid-October. After the blackout period, you can make back claims for anything done September 1st or after. Opt-Out Details for Health/Dental Insurance

Students who have alternative equivalent coverage can opt-out of the SGPS health and/or dental plan.

How to Opt-Out

Between Monday, September 12 and Friday, September 23, 2011 go to http://www.studentplans.ca, choose "Queen's University" and follow the on-screen instructions.

You will be asked to provide Proof of Coverage including the Name of your Insurance Company, your Policy Number, and the name of the person who holds the policy.

There are no paper opt-outs
NO OPT-OUTS WILL BE ACCEPTED AFTER SEPTEMBER 23rd, 2011

Keep a copy of your opt-out confirmation email. This will be your proof of opt-out should you need it.

Before the end of September, successfull opt-outs will be processed and a credit will be applied to your Solus account.
